AAA HOME HEALTH SERVICES

Skilled Nursing services include, but are not limited to:
* Coordination and implementation of physician's plan of treatment.
* Routine patient evaluation and communication to physician regarding patients response to care, and changes in the patient's condition.
* Interaction with patient's family/caregiver, co-workers and other disciplines to provide observation and feedback regarding patient's condition.
*Review of medication regime and patient training.

Home care aide services include, but are not limited to:
* Maintaining accurate clinical records regarding all patient observations, care and treatment.
* Assisting patient with ambulation and/or transfer, using proper body mechanics to protect patient and personal safety.
* Assisting patient in meeting personal hygiene needs.
* Preparing meals and assisting with eating if necessary.
* Providing simple household procedures essential to health care at home.

Physical therapy services include, but are not limited to:
* Assisting physicians with initial and on-going patient evaluation.
* Directing and aiding patients in various exercises and activities.
* Gait training
* Instructing patient and caregiver in use of durable medical equipment, and prosthetic and orthopedic devices.
* The application of various devices, stimulators and traction.
* Coordinating physical therapy with home health agency's nursing services.

Speech therapy services include, but are not limited to:
* Coordination of plan of treatment with the patient's physician.
* Patient evaluations and communication with physician and nursing staff.
* Instruction to patient and caregiver in therapeutic exercise procedures.
* Psychosocial support for patient and family.

Occupational therapy services include, but are not limited to:
* Evaluating the patient's level of function.
* Guiding the patient in therapeutic, creative and self-care activities.
* Helping the patient adapt to age-related changes such as decreased energy and vision.
* Assisting patient to increase physical strength and endurance to maintain self-sufficiency.
* Identifying and reinforcing the older adult's abilities for independent living.

Medical social worker's services include, but are not limited to:
* Communicating with home health professional staff regarding patient's psychosocial assessment and collaborating with necessary sources to meet the patient's needs.
* Helping patient achieve independence.
* Diagnosing the severity of a patient's social programs and communication with physicians.
* Assisting patients and their families/caregivers to manage personal and environmental challenges which prevent the patient from fully benefiting from home care.

IV certified Registered Nurses on staff
I.V.Therapy services include but are not limited to the following:
*Antibiotics
*Chemotherapy
*Pain management
*Parenteral Nutrition
*Cardiac Drug Therapy
Wound Care Program
*Certified Wound Care Nurse Specialist on staff. *Consultation & inservices available for Wound Care
A Registered Nurse will be on call 24 hours a day 7 days a week to answer any questions or concerns.
